Seniors Outreach Teams - For Primary & Community Healthcare Providers

Seniors Outreach teams provide support to primary care and community health care providers when a senior needs further assessment and treatment, or to investigate abuse or neglect.
Seniors Outreach Teams work with the Seniors Outpatient Clinics to support primary care providers and community health care providers when a senior needs further assessment and treatment. Teams will liaise and provide navigation support based on the patient's needs, and can advise what services, assessments, and treatments would be beneficial. These can include areas such as mental health and substance abuse, age-related conditions and other concerns. Seniors Outreach Teams do not do hands-on assessments or treatments, but may be responsible for investigations of abuse and neglect.

How To Access This Service Primary Care Provider/Community Health Care Provider will contact their Regional Community Access Centre or Seniors Outpatient Clinic if their patient attends one (see below).
Note: This service is intended to support primary care/community health care providers. However, other individuals may contact the Community Access Line if assistance/information is required about this and other services.
