Linked to Treatment Program (L2T)

The L2T program provides support to access related treatments for individuals who have tested positive for HIV/HCV.
The Linked to Treatment program (L2T) aims to improve the quality, effectiveness, and reach of HIV prevention services by connecting individuals to care and treatment. L2T assists individuals struggling to access HIV/HCV care and treatment, or those who are newly diagnosed and would like support to access treatment and address lifestyle factors.

This 3-9 month program uses evidence-based interventions to promote and monitor treatment adherence. Individuals will receive intensive case management support through the process, receiving care, connection, and coordination for HIV/HCV treatment. As treatment regimens are complex and significantly impact health and wellness, L2T welcomes community partners’ involvement in a holistic, connected care team approach and in cases where there are physician referrals and coordination support for the referred person.

Program Eligibility
  • Newly diagnosed with HIV and/or HCV
  • Re-engagement into care for HIV and/or HCV positive individuals
  • HIV and/or HCV positive individuals who would benefit from strengthening their engagement into care
  • Able to provide legal consent; i.e. is older that 16years of age
