On Track Program - A Cardiovascular Health Program

A health education and supervised exercise program for people who would benefit from clinical support in making lifestyle changes.
The goal of the 'On Track' program is to provide evidence-based support to reduce cardiovascular risk and improve mental health and overall well-being. This program is for patients with established risk factors for cardiovascular disease such as Diabetes, Hypertension, Dyslipidemia, Metabolic Syndrome as well as mild to moderate depression and/or anxiety. Exclusion criteria are for people with a diagnosis of cardiovascular disease. The program is run in partnership with Interior Health.

Participants will gain skills in mindful eating, label reading, stress management, goal setting and action planning and cognitive behaviour techniques such as thinking realistically and problem solving.

Format:Sessions run weekly for 10–12 weeks with guidance from Interior Health and City of Kamloops staff. The program runs in the Fall (Sept.), Winter (Jan.), and Spring (April).

Cost:There is a program cost of $30. If participants cannot afford the program fee, they can receive a subsidy (of up to 80% of the fee) by applying to the City of Kamloops’ Affordable Recreation for Community Health program.

To qualify for the program, participants need to get a referral from their physician and go through a screening process (which is done by the Program Coordinator).
