Develops exercise routines for individuals with chronic health conditions.
Receives referrals from GP’s and NP’s, PCN nurses, social workers, physiotherapists, OT’s etc to aid individuals with diabetes, heart issues, COPD and CHF to get an exercise routine going. Sessions are provided on a 1:1 basis.
Service is available in English.
