Heatley Community Health Centre - Primary Care and Mental Health Support

Provided by Vancouver Coastal Health

Heatley Community Health Centre brings together existing programs and services so clients get the care they need at one location. Primary care, mental health and substance use services, harm reduction and specialized care are all available on site.
Services include individualized mental health screening and assessment; rehabilitation goal–setting; basic support (access to activities); rehabilitation readiness assessment (exploration of service options, building confidence); rehabilitation process (assistance with goal achievement) ongoing support, referral to other services and advocacy.

Integrated Care Teams (ICTs):
ICTs serve the marginalized people of the Downtown Eastside neighbourhood who are adversely affected by the social determinants of health, have no primary care provider and are challenged by complex health concerns (including psycho-social, physical, mental health and addiction co-morbidities). Each ICT provides care on-site and via outreach, with nurses, social workers, counsellors, peer specialists, community liaison workers, family physicians, nurse practitioners, and psychiatrists. ICTs are supported on-site by more specialized care providers: wound care clinicians, palliative care nurses, occupational therapists, dieticians, and many other disciplines. A client will be connected to a specific ICT for ongoing care.
