Seniors Health & Wellness Centre - Central Okanagan

Seniors Health and Wellness Centre patients are referred by their family doctor, nurse practitioner, or specialist. Please see referral form below. Your healthcare provider can then fax it to: Central Intake at: 250-980-1505
Services Include:
  • Inter-professional team providing short term assessment, education and support for seniors struggling with: falling, memory changes, nutrition, pain, caregiver support, medication concerns, speech or swallowing difficulties, bowel or bladder concerns
  • Health care team includes: Geriatrician, family physicians with a special interest in geriatrics, mental health clinician, physiotherapist, occupational therapist, registered dietitian, rehabilitation assistant, speech and language pathologist, pharmacist, registered nurse, social worker, and respiratory therapist provide specialized short-term care (four to eight weeks).

  • Clients must live between Peachland and Lake Country, BC.
  • Criteria:
  • CSHS Clinical Frailty Scale: frailty scale of 5-6
  • Complex (at risk of decline without interventions)
  • Geriatric Syndromes: pain, nutrition/hydration, cognition (delirium, dementia, depression), mobility/function, incontinence: bowel/bladder, medications.
  • Agreeable for treatment
  • Potential to stabilize/improve
